401(k) Eligibility Requirements: You become eligible to participate in the matching contributions program after completing one (1) Period of Service and reaching the age of 21. However, you will begin participating in matching contributions when you reach the Entry Date.

Matching Contribution: The Employer may provide a discretionary matching contribution, which can be either a uniform percentage or a fixed dollar amount of your elective deferrals each Plan Year. The specific formula for the discretionary matching contribution will be determined by the Employer each year.

100% Vested Contributions: You always have full ownership (100% vested) of the amounts in your accounts that result from the following contributions:

  1. Elective deferrals, including catch-up contributions.
  2. Rollover contributions.
  3. Matching Contributions: Your vested percentage in your account stemming from matching contributions is calculated according to the schedule provided. However, you will always be 100% vested in your matching contributions if you are employed on or after your Normal Retirement Age, or in the event of your employment termination due to death or disability.

For Flying:

The U.S. Nursing Housing and Travel department handles the coordination of air transportation for your assignment. Once you’ve accepted the assignment offer, our team initiates the process of booking your travel arrangements. A U.S. Nursing representative will then contact you to provide your itinerary, or it will be sent directly to your email.

For Driving:

Should you decide to drive to your assignment, U.S. Nursing will calculate mileage reimbursement based on your home address, which will be included in your first paycheck.

U.S. Nursing offers a courtesy shuttle service to transport you to and from the facility for every shift. Alternatively, you can choose to transport yourself to the facility during the work stoppage. If you opt for self-transportation, it’s essential to check in with a U.S. Nursing representative upon your arrival at the facility for each shift.

The travel dates for each job action will be communicated during the recruitment period.

As housing availability is frequently limited during a job action, we regretfully cannot accommodate additional family members or pets. However, service animals are acceptable with the provision of appropriate documentation and prior approval from U.S. Nursing.
